If you are responsible for evaluating other foreign teachers you can use the following form.

The following are the criteria on which the instructor is evaluated:

Accomplished his/her duties on time
Learns from previous mistakes
Confronts all responsibilities
Tries to finish all responsibilities within the required time period Cooperates with subordinates, colleagues and superiors
Becomes emotionally attached to work
Instructed with a bright and lively attitude
Ability to be present and on time for all classes
Doesn’t let private issues affect performance
Maintains a neat and orderly classroom and office area
Provides enthusiastic lectures
Developed a stable classroom environment
Deals with classroom disruptions and issues in a timely manner
Consulted and communicated with staff quickly and precisely
Evaluated and recorded students’ test scores and attendance
Understands duties
Conducts duties in a timely manner
Accomplishes duties undertaken
Cultivated a habit of thinking for oneself
Takes time for professional development
Capable of applying knowledge and techniques in the classroom
Tries to improve the image and quality of the school

Although this post is quite old, it has proved its usefulness because it contains exactly what I've been looking for. Thank you for posting.

This looks good, do you also solicit an evaluation from the students to see how they feel the course was? If so how do you feel it works?
