This is my ESL Lesson plan template, which i always used for my lesson, what should i do, the objectives, materials prepare. We are required to fill in everything during my practicums and have to gave a copy to my assessor too..
Hope it's helps for your lesson, i always do that for my class and i keep that most of the time πŸ™‚

p/s: you need to used word text to fill in all the information.

Awesome forms!
I was wondering if you could provide a sample filled-out form, especially the second one. I am curious what you, jellybean, put in the "Context", "phase", and "interaction". Thanks!

context - the day's topic, i.e. colors/numbers/Halloween lesson etc.
phase - either S for "study" (when you introduce new words, for example) or A for "activity" or "engage" when you sing a song or do something to get your students' attention
interaction - either T-S for some teacher to student interaction (e.g. drilling new vocabulary) or S-S when the students get to interact, for example a role-play or something.
