We use this form to keep track of attendance, the professor(s) who is/are teaching the classes and the material covered during the classes.
There is a Spanish version and an English.

I have used some of these templates also. I use the attendance and homework sheets the most. They are very useful around the classroom. It is a very pleasent way of keeping track of missing homeworks which brings my students grades down.
