student's assessment feedback form

Need a few ideas for questions that could be useful to the teacher as feedback from the students about an exam they just took.:(

Ask students if they felt that the answers to the questions on the test were covered in class. As them if any of the wording in the test confused them. These are the two biggest issues I usually have in regards to testing. Good luck.
