Do you ever find that those sticky notes you post onto your director's computer just don't stick?

Or maybe you've noticed that no matter how many times they say "yes", the board markers you asked for just never show up?

Perhaps your request for "3 boxes of pencils" came back as 3 pencils! Then again, it could have been you who forgot to say how many!

Well, here is a simple, non-verbal alternative for making supply requests.

This form allows you to tell how many of that thing you want, for what reason, by when and the expected cost in blocks of 5 items (so as not to overwhelm anyone) making it decidedly easier to ask for things clearly!

Hi, Little Sage, had a look at you supply list.I'm going to make one that lists the supplies needed daily, or that runs out the fastest,eg. pencils, pens, markers
and all the person has to do with the page is check mark the quantities.
eg. pencils...3 ... boxes....by(date)...and...teacher's name.
what do you think?
